Tsunade continued to do her part in ending the war by repeatedly countering Chiyo's poisons from Hidden Sand Village . Tsunade confidently says that she doesn't care about the village, and Jiraiya remembers how important Konoha was to Nawaki and Dan; to top it off, he remarks how similar they, especially the Nawaki, are to Naruto. Tsunade meets with Orochimaru and agrees to heal his hands, but only on the condition that he leave Konoha alone. Orochimaru doesn't mind. Orochimaru asks why she deceived him, to which the "Princess" confidently states that Orochimaru was lying about sparing Konoha. Orochimaru decides to personally kill Naruto with the Kusanagi sword . Tsunade becomes a barrier, protecting Naruto, the future Hokage of Konoha. After using too much chakra during the fight, Tsunade loses her youthful appearance, but she recovered before Naruto could recover. Jiraiya planned to take Naruto outside the village with him and start training. In the anime, while Jiraiya was busy with his investigations, Tsunade loaded Naruto with missions to distract him from going after Sasuke. So, during one of the missions related to the Legendary Stupid Brothers ' prison break, Tsunade intervened and convinced the criminals to surrender by displaying her genjutsu and demonstrating her strength.

Tsunade received word of Naruto's return to Konoha after two and a half years of training. She shared the news with Sakura and they went to meet him together. Tsunade reactivates Team 7 under Kakashi. She initially tried to send them on a low-ranking mission to spite Naruto, but received a report that the Fifth Kazekage Sunagakure had been kidnapped by Akatsuki. Tsunade then decided to send Team 7 to rescue the Kazekage, realizing that the Akatsuki also wanted to capture Naruto. She was not initially worried, but when she won the lottery, she took it as a bad sign and sent Team Guy to provide more help.
A few days later, word reached her that the Kazekage had been successfully rescued and Team 7 and Guy were heading back to the village. Shizune asked if it was logical to send Naruto to fight the Akatsuki, to which Tsunade replies that since Naruto and Gaara are both jinchūriki, Naruto was the best person for the mission.
After Team 7's return to Konoha, Sakura revealed that they had the opportunity to meet with Orochimaru's spy and thus find Sasuke. Kakashi was bedridden as a result of the mission to Suna because he had overworked himself, so the question of adding new members to the team arose. Shizune started bringing up the subject of Naruto again, thinking it would be safer to keep him in Konoha. Tsunade, on the other hand, insists that Naruto is one of the few people she can trust with the mission with absolute certainty, and besides, this is his own business and he will not sit on the sidelines.
Soon after, Tsunade was summoned by the Leaf Council , to whom Shizune had already expressed her concerns and who also agreed that Tsunade's behavior towards Naruto was irresponsible. Tsunade stands his ground and adds that the Akatsuki will come after Naruto one way or another, and sending him on missions will make their search more difficult. The Konoha Council calmed down after Tsunade promised to personally take responsibility if her decisions resulted in Naruto's capture or endangered Konoha. But, they insisted that one of Danzō Shimura 's subordinates , Sai , become the third member of Team 7. The temporary leader of the Tsunade team is made by an Anbu native codenamed " Yamato»; partly because he was her confidant and would keep an eye on Sai, but also because his wood element could help Naruto keep the Nine-Tails under control.
In fact, while Team 7 was able to meet Sasuke, they were unable to get him to Konoha. This didn't stop Naruto and the others, and they promised they would try again. Tsunade didn't protest.
In the anime, Tsunade sent Team 7 to investigate a case of grave looting near the Fire Temple . When they returned, they reported that the robbers , whoever they were, seemed to be interested in the graves of members of the Twelve Protector Ninja . Since Sai was damaged during the initial investigation, Sora was assigned to help them in the Firelink Temple. Tsunade received secret messages about Sora's past and his unusual power that poses a danger to the Firelink Temple. At the same time, Tsunade takes Danzō into custody on suspicion of collaborating with Amegakure. The advisers advise either to limit Sora in actions, or to eliminate it altogether. While Tsunade was making her decision, Sora attacked her, but Shizune and Naruto arrived. Although one of the squads was able to reach the Akatsuki members ( Hidan and Kakuzu ), they escaped capture and killed Asuma Sarutobi. His corpse was taken to Konoha and a funeral was held while the rest of the squads continued their search.
Asuma's students, Team 10, left Konoha, joining the search and seeking revenge. Tsunade tries to stop them and, among other things, explains that they need a fourth member. Kakashi volunteers to take part and Tsunade surrenders. She realizes that they will need reinforcements and begins looking for candidates. Tsunade turns to Sakura, asking him to tell Naruto that if he wants to go, he must first complete the training on his new technique. And Naruto is really mastering the technique and the Akatsuki members have been defeated. Tsunade performed an autopsy on Kakuzu's body, assessing the damage done by Naruto's new Rasenshuriken . While examining the damage to Kakuzu's cells, Tsunade comes to the conclusion that Naruto's Rasenshuriken is too dangerous and asks him not to use the technique again.
In the anime, Tsunade receives a report from a dying Anbu member about one of Orochimaru's bases. She puts Kakashi on Team 8 and sends them to investigate. Since there was a possibility of meeting Sasuke, Tsunade keeps the fact of the mission a secret from Naruto. But he finds out about everything anyway, and Tsunade allows him and the rest of Team 7 to join as a backup. Jiraiya came to Tsunade and informed her of the rumors that Orochimaru had been killed by Sasuke. They told about this to Naruto and Sakura, who believe that Sasuke will go against his brother, Itachi Uchiha of Akatsuki. Naruto suggests that they find Itachi so they can find Sasuke. Despite all previous attempts to capture the Akatsuki members failing, Tsunade approves the formation of an eight-man squad and sends them out to find Itachi.

Tsunade feels guilty for always putting dangerous work on him and reflects on the happy times when they were still friends with Orochimaru, and also offers his help. But Jiraiya refuses. Jiraiya prepares to leave and Tsunade asks him to return alive, fearing that he might die because of her.
Shizune senses that Tsunade is worried about Jiraiya and tries to calm her down, reassuring her that everything will be fine. Tsunade replies that he will not return, thus choosing a new tactic for herself: she bets on what she really does not want, wishing in this way, with an eye on her bad luck, that her bet did not work.
In the anime, Konoha receives a report that the Tsuchigumo clan has been attacked and, as a result, their forbidden technique has been compromised. Since the eight-man squad was closest to the scene, Tsunade sent Katsuya to intercept them and instruct them to protect Tsuchigumo.

Fukasaku , Jiraiya's former teacher, reports to Tsunade that Jiraiya has died while investigating the Akatsuki leader, Pain . Tsunade called Naruto into her office and Fukasaku shared the news with him. Naruto became very upset and started blaming Tsunade for letting him leave. Sakura tried to stop Naruto, but Tsunade herself did not interfere. She continues to investigate Pain's identity using the information left by Jiraiya: she ordered Shizune to examine the body of the Animal Path ; she orders Ibiki Morino to interrogate the prisoners captured by Jiraiya; she tasks Shikamaru Nara with deciphering Jiraiya's posthumous message. When Shikamaru starts to object, Tsunade ignores him and, left alone, mourns Jiraiya.
Knowing that Pain would come for him, Naruto went to Mt. Myōboku to train with Fukasaku. The investigation into Pain's identity continued, but he attacked Konoha before the case was completed. Tsunade decided to send an order to Naruto to head back to the village, but was stopped by the Leaf Council, who felt that Naruto would be safer there. Tsunade got angry at the councilors and scolded them for not showing even the slightest bit of faith in Naruto's ability to protect himself. The councilors are offended by her manners, but still allow her to do what she wants. Afterwards, Tsunade climbs onto the roof of the Hokage's residence and summons Katsuya, asking her to split up and attach herself to every villager so that she can heal everyone at a distance.
After Katsuya has spread throughout the village, Tsunade begins to fully appreciate the damage caused by Pain's attacks. Learning through Katsuya of Kakashi's death, Tsunade destroyed the spire out of grief. She was eventually discovered by Pain's Deva Path . The Anbu detachment came to the defense of Tsunade, who, in the Deva Path, remembered an orphan who was once trained by Jiraiya. The Deva Path asked Tsunade about Naruto's whereabouts, however she refused to answer. The Deva Path then accused Konoha of doing things that other villages were in a sorry state of doing and of refusing to cooperate with Akatsuki. Tsunade remained adamant, but Pain was already able to find Naruto in other ways. Before leaving, the Deva Path stated that he would show Konoha the pain they had caused everyone else for so long.

The Deva Path takes off high into the sky and Tsunade tries to follow it. He is about to destroy the entire village with the Shinra Tensei , however, Tsunade transfers all of her chakra to Katsuya to keep the villagers safe. As the dust settled, Tsunade discovered that the village was gone. She confronts all of the Six Paths of Pain and promises to deal with them personally. The Asura Path attacks her, but Naruto arrives in time and asks Tsunade to entrust the victory over the Pains to him. Having done everything that was in her power and completely wasted all the chakra, Tsunade fell into a coma.

A lot of time passed, but Tsunade still came to her senses. Shizune immediately threw herself into her arms. After eating enough to replenish her chakra reserves, Tsunade learned about everything that happened while she was in a coma: Naruto defeated Pain, Akatsuki's Tobi announced the Fourth Shinobi World War , the Allied Shinobi Forces were formed . Kakashi discusses with her all the problems that have piled up and thanks her for coming to her senses and now he will not be appointed Hokage. A few days later, Tsunade traveled to Kumogakure to meet with the rest of the Alliance's leaders and discuss topics of wider concern. On the agenda was the question of what should be done with Naruto and Killer Bee , for the sake of capturing whom Toby unleashed a war. The other Alliance leaders had already made the decision to isolate them, and only Tsunade objected, believing they would be invaluable on the battlefield. The Fifth Kazekage, however, retorted, reminding him that Naruto has a tendency to act recklessly for his friends who risk themselves for him. Realizing that no one would listen to her opinion, Tsunade had no choice and she reluctantly agreed that Naruto and B should be sent to Turtle Island.. In the anime, after meeting with the leaders of the Alliance, she asked B to teach Naruto how to control the Nine-Tails in light of recent events, which was approved by the Raikage . When it came to other matters, Tsunade took a particular interest in equipping the rear medical company with everything needed.
In the anime, Team 7 is sent by Tsunade to investigate a massacre in the village of Tonika . When Team 7 informs her about Kabuto Yakushi's use of Summon: Impure World Resurrection here , Tsunade sent all of her reserve forces to them.
In the anime, Tsunade sent Sakura to Benisu Island to collect medicinal herbs that could be used for military purposes. She later prevents Konohamaru Sarutobi from joining the Shinobi Alliance.
Most of the Alliance leaders went to the battlefield when the fighting began, but Tsunade and the Fourth Raikage remained at headquarters to deal with tactics, coordinate troop movements and receive new information. On the first night of the battle, they received a report that Naruto and B had managed to escape from Turtle Island. The Raikage takes Tsunade with him, intent on stopping the jinchūriki; before leaving, Tsunade left a part of Katsuya at headquarters so that if something happened, she could contact her.
As soon as they found Naruto and B, the Raikage stopped them and began to convince them to go back to Turtle Island. Naruto begged Tsunade to let them through, but she explained that she must do what was in the best interests of the Allied Shinobi Forces. Naruto continues to persist and encourages the Raikage to make the decision to kill him so he doesn't fall into Akatsuki's hands. Tsunade does not allow this and comes to the youth's defense, noticing that the new Nine-Tails Chakra Modeindicates his unparalleled control over the Nine-Tails, which may not be able to be replicated by any of the next Nine-Tails jinchūriki. Tsunade thinks the best way out is to let Naruto use his new power and win the war. The Raikage doesn't agree with Tsunade, much less B and Naruto's words, and seemed to be ready to fight them, but when Naruto managed to slip past him, he didn't pursue him.
When Tsunade and the Raikage returned to headquarters, they were informed that the White Zetsu Army was made from the DNA of Hashirama Senju. Tsunade is annoyed that her grandfather's powers continue to be used and that Tobi could still use them, but trusts that Naruto will be able to eradicate the Zetsu that had infiltrated the Allied Forces.
Naruto, using his shadow clones, was indeed able to help defeat the White Zetsu by detecting those who disguised themselves as Alliance shinobi, which was beyond the power of a normal ninja, and, moreover, assisted in capturing the remaining resurrected enemy forces. The turning point was interrupted by the sudden appearance of the resurrected Madara Uchiha on the battlefield of the Fourth Division . Upon learning of this, Tsunade expressed a desire to join the battle: Mabui transferred her to the place where the battle with Madara took place using the Sky Movement Technique , in parallel with which the Fifth used Divine Regenerationin order to restore your body from the damage that such a movement technique carries. The Raikage joined her and they arrived in time to save the Fifth Kazekage and the Third Tsuchikage from Madara's attack. Tsunade began to heal her colleagues, and then, upon the arrival of the Fifth Mizukage , all five Kage came face-to-face with Madara. Confident in their victory, they told one of Naruto's shadow clones to leave Madara on them and go on his own to defeat Tobi.

At first, Tsunade played a supporting role, healing the other Kage from taking damage and, as needed, replenishing her own chakra reserves. She was extremely surprised when Madara manifested her grandfather's Wood Release abilities, but it turned out that he implanted the face of a Hashirama clone into his body and gained access to all of his abilities . Madara realized that Tsunade was Hashirama's ancestor and decided to kill her first, mocking her for being so much weaker than her grandfather. She admitted that she did not inherit Hashirama's natural gift, but she does have the Will of Fire , which prompted her to create the Ninja Art Divine Regeneration - Hundred Strength Technique.. Activating the technique, she joined the rest of the Kage in attacking the enemy. Madara was unimpressed by this, but Tsunade was still able to strike at his body, giving the Kazekage a chance to try and seal the ill-wisher.

Madara eludes capture and stabs Tsunade with Susanoo 's sword . Madara believed that she was dead, but Tsunade got rid of the sword and went against him again, admitting that she may indeed have inherited something from Hashirama. Madara admits that the Kage managed to give him a beating, but states that it only happened because he was in a five-on-one position; wanting to act "honestly", he created twenty-five tree clones, five for each Kage. The tree clones started using Susanoo and clashed with the Kage. Despite Madara's numerical advantage, the Kage were able to combine their forces against the original, defeating him even through his completed Susanoo form. Before he could destroy the Kage, the Impure Resurrection technique was dispelled and Madara's soul began to return to the Pure World.

Madara uses his remaining time intent on finishing off the Kage. Tsunade takes his attacks on herself, relying on her Divine Regeneration, but she runs out of chakra. Unexpectedly, she was saved by the Spirit Transformation TechniqueDana Kato, whose soul was also freed from the Unholy Resurrection. Tsunade used this opportunity to apologize for turning away from his dream after his death, but now she has changed and supports his ideals by becoming Hokage. Dan thanked her for this, but said that he would do him a great favor if he remained alive, as she is in his dreams. Dan kissed her on the forehead, and then disappeared, transferring his chakra into her. Orochimaru retreats while Tsunade stays behind to complete the Kage's healing.

After all the Kage came to their senses, the Kazekage moved everyone to the site of the Allied Forces' battle; along the way, Katsuyu briefed them on what had happened while they were out of their minds. Approaching the battlefield, they, like the rest of the shinobi, were addressed by the resurrected Hashirama Senju. After greeting Tsunade, Hashirama explained the purpose of the God Tree : to lead to the Infinite Tsukuyomi . Out of fear, most of the ninja began to thrash about chaotically in fear, but after the arrival of the five Kage, they calmed down and joined forces. Tsunade located Sakura and teamed up with her, summoning a large portion of Katsuya spread all over the perimeter, thus they could continuously restore everyone in battle.
Although they were unable to get rid of the God Tree, the Union forces helped Naruto defeat Tobi, whose real name was Obito Uchiha . Naruto leaves to fight Madara, leaving the Allied Forces to fight another Zetsu . Tsunade lacks the chakra needed to participate in the battle, much less continue to perform medical ninjutsu. Therefore, when the Kazekage brought Naruto to them in need of urgent care, Tsunade sent Sakura to help.

Despite the Allied Forces' best efforts, they were unable to stop Madara from activating the Infinite Tsukuyomi, which took the entire world into a dream. Inside the Infinite Tsukuyomi, Tsunade dreamed that she was in Konoha, where Dan is the Hokage, Nawaki and Jiraiya are alive, and Orochimaru did not leave the village. In the anime, while waiting for Dan, she read a novel written by Jiraiya called Jiraiya's Ninja Scrolls: Tale of the Hero Naruto. In the novel, there were far fewer upheavals in Konoha: the Fourth Hokage and his wife survived the attack of the Nine-Tails, and the destruction of the Uchiha clan did not happen. Naruto and Sasuke eventually dispelled the Infinite Tsukuyomi, freeing Tsunade and the others, thus ending the Fourth Shinobi World War.
Like the other students of the Third Hokage , Tsunade is an exceptionally talented shinobi. Even before the Second Shinobi World War . On the forehead there is a symbol in the form of a diamond - Byakugo no Jutsu.
Tsunade has excellent control over her chakra, a necessary prerequisite for her role as a medical ninja . After the Second Shinobi World War, she began storing her chakra in the Byakugō .
