A communications system can be provided for communicating patient information among health care providers, who may include clinicians, doctors, interns, residents, physician's assistants, nurses, or any other health care personnel. The communications system can enable tracking of the responsibility relationships between changing providers and patients. In addition, the communications system can allow rapid and reliable messaging between any provider and the provider responsible for the patient at any one time.
